- Graduate from a recognized Physical Therapy college or university.
- Hold a valid Physical Therapy license for New York.
- Have CPR Certification.
- Know how to use EMR documentation (experience with Hello Note is unnecessary).
- Be able to do home visits for Physical Therapy assessments.
Responsibilities
- Visit patients at home to check their health and abilities.
- Provide care and create personal rehab plans for each patient.
- Record patient progress and treatment in electronic medical records (EMR).
- Work with other healthcare workers to give complete care to patients.
Salary And Benefits
